Clerks, keep an eye out for those big white envelopes in the mail. If you already know your login info, you can go ahead and complete the state portion of the ACP now. If you’ve forgotten your login, just send me an email and I’ll get it to you. Everything is being set up online this year, with paper copies available as backups for anyone who prefers to use paper forms.

The following forms are for associational use. These will take the place of our normal paper forms. I will place the paper forms in your packet for those who prefer to use those as well. But we ask that all information be entered online to streamline the process this year. If you are not comfortable with computer forms, feel free to complete the paper forms and bring them to me at the office and I will enter your information.
